Ealth Promotion Plan on Substance Abuse

Target population: teenagers 13-14 year olds

Develop Health Promotion Teaching Plan using the Nursing Process format. Under the introduction portion of your paper, Identify 1. A Problemyou have noticed and why YOU think this is a problem for the group YOU have selected to write a health teaching plan on. Ex. Why this is a problem for the aggregate group (i.e. adults, adolescence, elderly, children) you have chosen. 2. Provide Supportfor your problem from the literature you will read for the problem and aggregate group you have selected by citing quotes. 3. Please make sure that your problem and Nursing Diagnosis stems from the literature. 4. Make sure to include short term and long term goals for your plan, but that goes in the body of you paper not in the chart. 5. Then list a plan, intervention you developed, rationale and 6. Finally, develop an evaluation process to ensure that your interventions achieved or not achieved your goals.

What I basically need is a 5-page paper presenting a teaching plan on substance abuse for teenagers. In the paper I also need to include where I will teach my target group and how I will get a permission to do that (I was thinking to do it at a school and ask the principal for permission by writing a letter explaining what I will do and why [you dont need to actually write the letter just talk about it in my paper]). I need to include researched information on the problem (as stated above) etc. I also need that chart that my prof. is asking for including the plan, intervention, rationale, and evaluation. The plan to get the permission from the principal needs to be included as well in the chart, so it will look something like this: Plan: Get permission to teach a class on substance abuse by writing a letter to the schools principal, Intervention: Wrote a letter to the Principal, Rationale: Need permission from principal to teach her students, Evaluation: permission granted”…. and then move on to how you will actually teach the teenagers THIS ALL NEEDS TO BE IN A FORM OF A CHART.
