Formal complain letter about my co-worker

i am a sales person and i work for a furniture company. i wrote a letter to my general manager to complain about my co-worker who has been extremely rude and trying to get me fired. he even complained about me today and accused me of threatening him to my manager even though i do not talk to him he wants to retaliate and tries to find ways to put in troubles. please mention this in the letter everything i want to say is in the letter please see attach file please rewrite in very professional way and organize my thoughts in very effective way
