The lady is 55 and of Afro-Caribbean background. the patient has good sight and hearing but can tend mumble words in Patois a Jamaican dialect. needs assistance with personal care on a daily basis. Suffers from Schizophrenia, query dementia. cannot communicate well due to cognitive impairment. Medication: Citalopram tablets 2mg, Levothroxine tablets 100mcg, olanzapine tablets 2.5 mg night time.

patient is religious used to attend Seventh Day Adventist, used to attend afro-caribbean community initiative centre. stopped attending for some time. possibly because no one had time to take her. it has been documented that probably she became more unable to communicate as she had no one to talk to in her own language. at times she is often confused and disorientated .

she can nod and dance to christian music shows that she understand some things. she was awaiting mental health review assessment but keeps postponed as the doctors cannot communicate with her effectively. she has got no relatives in the UK and she lives in a mental health institution.

came in the UK in the 1990s and at the time could communicate with difficulties as english is not the first language. diagnosed with schizophrenia after she was found outside of her hotel building undressed and unaware of her name and place. there is a possibility that the medication contributed to her communication skills..

for references can you please use cochrane, medline, cinahl databases… also NMC Code, Department of Health, Human rights act, nursing standard etc.
