(the care plan is the essay that i did for the first part of this assessment which i will attatch further down.....i will also attatch the handouts that i have received for lectures on this module as i think they may be of help)

Guidelines

the essay should include:

1. Discussion and definition of health promotion.

2. Selection and application of how a model of health promotion can be applied in practice

3. Discussion of how governmental policies influence practitioner s activities in health promotion

4. Demonstrate evidence of consultation with the patient to identify their health needs and desire for health promotion

5. Provide relevant and current health information to the patient in a form that facilitates their understanding and acknowledged choice/individual preference

6. Demonstrate an understanding of the influences on health

7. Identify and demonstrate an understanding of multi-agency roles in promoting health.

8. You should analyse the links between theory, research and practice

9. Links must be made to appropriate clinical practice experience

10. You should include, as an appendix, a copy of the Proficiency 2.2. Create and utilise opportunities to promote the health and well-being of patients, clients and groups. This aspect must relate to your health promotion topic and be referenced with appropriate supporting literature. It is not appropriate to write in a descriptive and anecdotal manner.
