Submit a 3000 word essay on your chosen long term condition (70% of total mark awarded).

The patient journey includes the following stages;

1. Diagnosis,
2. Living for Today,
3. Transition/Progression
4. End of Life.

You must choose one of these stages of the patient journey as the main focus of your essay. You are also required to include a brief summary of the remaining three stages of the patient journey.

Within the essay explore and discuss the Health care professionalsi?? role in providing holistic practice throughout the patient journey. Discussion must be supported with reference to current literature and evidence based practice guidelines.

The stage of the journey you have chosen to focus on is an opportunity to discuss some issues in-depth. For the remaining 3 stages of the patient journey you are only expected to discuss the key issues.

Please refer to marking criteria for component 1 and 2 in appendix 1, 2 and 3.

Word Count: the word count includes all the text from the beginning of the introduction to the conclusion/recommendations and the references included in this text. The word count does not include appendices, reference list or bibliography. Please note that if the word count excluding contents page, reference list and appendices is more than 10% over the word limit (3,000 words) then you will be penalised. Please check table below for information on the penalties which will be applied.

Introduction –

Set the scene for the essay by including the relevant areas to be addressed. Within this you should include your choice of long term condition, which stage will be the main focus of the essay and your literature search strategy. (Approx. 200 words).

The main body –

The main body should include 3 sections

1. A brief outline of the chosen long term condition including pathophysiology and incidence. (approx. 200 words)

2. An in-depth exploration and discussion of the Health care professionalsi?? role in providing holistic practice at one stage of the patient journey. This can be either, diagnosis, living for today, progression/ transition or end of life. Discussion must be supported with reference to current literature and evidence based practice guidelines. (approx. 1500 words)

3. A brief exploration and discussion of the Health care professionalsi?? role in providing holistic practice throughout the other three stages of the patient journey. This section should only include the key issues for your chosen condition. (approx. 250 words for each of the 3 stages)

Conclusions and Recommendations –

This is a summary of the key points that you have raised within the essay. Demonstrate that you have explored the patient journey as set out in the introduction. Identify how the new learning gained will inform your future practice in relation to providing a holistic approach to long term conditions.
