Collaboration in practice a case scenario
Dear Students

For your formative assessment taking place on 2nd May 2014 you will be required to work in small groups to complete a role play exercise. You will be required to demonstrate collaborative working as members of the Multi Disciplinary Team (MDT). Within the role play each group will demonstrate the discussions, viewpoints and decisions of particular members of the MDT during a MDT Case Conference.

Scenario
Sarah is 16 years old and has been recently admitted to the Accident and Emergency (A+E) Department via her GP for hyperglycaemia. On admission to A+E Sarahas mother reported that Sarah has lost a lot of weight. In addition Sarah appears to be depressed. She is not spending time with her friends as she prefers to be by herself. The family have noted that Sarah is displaying very marked mood swings and are becoming more concerned. They raised concerns surrounding her poor compliance to treatment earlier this year. The nurse triaging Sarah in A+E is unsure of the appropriateness of the GPas referral. The nurse believes that it would be a more appropriate use of resources for Sarah to be managed in the community. Sarah is well known to staff on the paediatric ward, but there are no beds available on the Paediatric Ward so the nurse in charge on the adult endocrinology ward has been contacted to arrange a possible admission.
A Multi-Disciplinary Team Case Conference has been called to collaboratively identify Sarahas needs.
a) You are the members of the Multi-Disciplinary Team (MDT) Case Conference and need to identify the needs involving Sarah and her family.
b) One member will be the team leader facilitating the Case Conference a you need to identify which practitioner would take on this role.
c) The remaining members of the group will take the role of one member of the MDT.
d) At the end of the role play, you as MDT team members will have identified Sarahas needs and have highlighted the difficulties faced by professionals who work in collaboration.

OUR GROUP FEEDBACK FROM SARAH AND HER MOTHERPOSITIVE FEEDBACK--GOOD TEAM WORKING AND COLLABORATION, GOOD THE FACT THAT WE MAINTAIN EYE CONTACT WITH PATIENT AND THE OTHERS, GOOD CARE PLANNING, GOOD EXPLANATION OF THE CARE , GOOD WAS THE FACT THAT WE INVITE SARAH AND HER MOTHER TO TAKE PART AT THE MDT MEETING , THE NEGATIVE FACT WAS THAT WE HAD A SOCIAL WORKER PRESENT WITHOUT EXPLAINING PRIOR THE MDT MEETING HER PRESENCE AND PURPOSE.
